To mark Type 2 Diabetes Week there will be a dedicated event held at Hanson Lane Enterprise Centre to raise awareness of the increased risks of the condition in the South Asian Community and offer guidance on building a healthier lifestyle.
Organised by Calderdale Cares Partnership and the NHS West Yorkshire Integrated Care Board, the event will shine a spotlight on the risk factors of type 2 diabetes, as well as the non-clinical measures individuals can take in reducing these risks and preventing the onset of type 2 diabetes.
At the event a number of partners will be present, including:

- Sign posting

- Blood Pressure checks
- Sign posting to Reed Wellbeing
- Access to all 4 practices systmOne, so therefore could task referrals to NDPP if individual has a HbA1c reading between 42-47mmol within last 12 months
- Signpost individuals back to practice for updated HbA1c and referral to NDPP
